Transitional Advocacy

When a teen ages out of the children’s system, tragedy can ensue if there is not a plan for transition to adult life. We provide assistance in time of crisis and also to prevent crisis. We assist teens who are eligible for the system to get a slot assigned at a Community Centered Board and to locate supported living services. We specialize in community-based options for appropriate mainstream living, but we also serve people who choose segregated settings.

Our transitional advocate assists the focus person in voicing their desires and concerns. Should a teen be eligible for the system, Individual Plan’s are created by everyone concerned with the well-being of the focus person along with service providers such as community centered boards. Because such plans set responsibilities for health, housing, safety and quality of life issues, they lay the foundation for healthy, full lives.
