New ID Requirements:
As of Thursday, December 14, 2006 there is a preliminary injunction staying the new ID requirements.
The ID requirements that are "on hold": The new citizenship requirement affects Medicaid applications and re-determination for current recipients. The new regulations do NOT apply to:
CHP+ or SSI recipients
Newborn children of eligible mothers
Foster children
Non-citizens
There are two methods of satisfying the new ID Requirements:
Method 1:
Provide one of the following documents:
Certificate of Naturalization (DHS Forms N-550 or N-570)
Certificate of U.S. Citizenship (DHS Forms N-560 or N-561)
Method 2:
Provide one document from list A, and one document from list B:
List A
U.S. birth certificate (It normally takes 14 days or more to get at birth certificate.)
Report or Certification of Birth Abroad of a U.S. Citizen (form FS-240 or FS-545)
U.S. Citizen I.D. Card (DHS form I-197)
Adoption Papers
Military Record
Affidavit
List B
U.S. Passport
Driver’s License or ID card
School identification card
Federal, State or local government identification card
U.S. Military identification card if it shows where you were born
If you do not have the qualifying documents, they can be obtained with the assistance of Metro Care Ring (MCR). MCR will need either an ID from any state (valid/expired/photocopy), birth certificate, social security card, passport, etc or two pieces of something that identify the individual. For example, two pieces of mail with their name on it. Appointments with MCR can be scheduled by calling 303-860-7200.
